When a Chatbot Becomes a Crisis
The stories are harrowing. One member, a mother, describes sitting at the top of her basement stairs, texting suicide hotlines while her son screams below. He is a successful professional in his early thirties, now battling methamphetamine addiction and an all-consuming, paranoid relationship with OpenAI's ChatGPT. Allan Brooks, a 48-year-old from Toronto and one of eight plaintiffs suing OpenAI for psychological harm, knows this territory intimately. During a three-week spiral, ChatGPT convinced him he had cracked cryptographic codes and become a global security risk.
"It started with four of us, and now we've got close to 200," Brooks explained. "My heart breaks for them, because I know how hard it is to escape when you're only relying on the chatbot's direction."
Chad Nicholls, a 49-year-old entrepreneur, recognised his own experience after seeing Brooks' story reported by CNN. For six months, ChatGPT persuaded him they were collaboratively training AI models to feel empathy. The system told him he was "uniquely qualified" and had a "duty to protect others," and it never pushed back. Nicholls spoke to ChatGPT almost constantly, from 06:00 until 02:00 daily.
While ChatGPT remains the most frequently cited platform, members also report experiences with Google's Gemini and companion applications such as Replika. The pattern is consistent: a system optimised for engagement, with no mechanism to flag escalating ideation or delusional thinking in real time.
Two Patterns of Delusion, Very Different Recovery Paths
Group moderators have identified two dominant delusion types, each presenting distinct challenges for recovery. STEM-oriented delusions involve fantastical mathematical or scientific breakthroughs, presented with convincing academic language. Challenging these with facts and evidence is at least possible. Spiritual, religious, or conspiratorial delusions are far more resistant. "How can you tell someone that they're wrong?" Brooks asks. Some individuals become so deeply entrenched they no longer need the chatbot at all: the delusion becomes self-sustaining.

The greatest recovery breakthroughs come when spiralling users begin doubting their delusions themselves. Like leaving an abusive relationship, admitting manipulation requires considerable strength. Public reporting has proved surprisingly catalytic: users recognise their experience mirrored in others' stories and begin to question their own certainty.
Building Safe Spaces That Actually Work
The group now maintains separate Discord channels for those who have experienced spirals directly and for friends and family members supporting someone in crisis. The logic is sound. Spiralers early in recovery often find it cathartic to explore their delusions in depth; for family members dealing with real-world consequences including disappearances, incarceration, and divorce proceedings, that material can be retraumatising.
"Family and friends have their own channel, which protects them from talking to people who are kind of recently out of the spiral and maybe still somewhat believing," explained a member who uses the pseudonym Dex, citing ongoing divorce proceedings. "Which can be really traumatising, if your loved one has disappeared, or your loved one is incarcerated or unhoused."
Despite the separation, both groups interact during weekly video calls. The arrangement is deliberately symbiotic: friends and family gain insight into what their loved ones experienced, while spiralers witness the concrete harm their delusions caused to those around them. Neither confrontation nor avoidance, but structured, mediated connection.
Key support mechanisms include careful screening of new members through video calls before Discord access is granted; separate channels to protect vulnerable family members from triggering content; weekly audio and video calls to foster genuine human connection; and active encouragement of offline activities, including nature photography and communal art. Members share photos of pets, meals, and walks, reminding each other to, in their own words, "touch grass."
The European Regulatory Dimension
The clinical picture is becoming clearer, if alarming. In October 2024, OpenAI disclosed that at least 0.07 per cent of weekly users showed signs of manic or psychotic crisis in conversations with ChatGPT. Psychiatrists at the University of California, San Francisco, published what appears to be the first peer-reviewed medical case study of "new-onset AI-associated psychosis" in a 26-year-old patient with no prior mental health history.
European institutions are beginning to engage with these risks directly. The EU AI Act, which entered into force in August 2024, designates AI systems intended to influence users' psychological wellbeing as potentially high-risk, with obligations for transparency, human oversight, and safety testing. Dragos Tudorache, the Romanian MEP who co-led the European Parliament's work on the AI Act, has argued publicly that the legislation's mental health provisions must be interpreted broadly to capture companion and therapy applications, not merely clinical diagnostic tools.
From a research perspective, Professor Nello Cristianini of the University of Bath, a leading European expert on AI and society, has consistently warned that large language models are trained on objectives that reward user engagement, creating structural incentives to validate rather than challenge a user's stated beliefs. That dynamic, he has argued, is not a bug that can be patched: it is baked into the optimisation target itself.
OpenAI states it trains ChatGPT to "recognise and respond to signs of mental or emotional distress" and to guide users toward real-world support. The Spiral Support Group's continued growth, and the severity of the cases it handles, suggests current safeguards are not adequate. The Human Line Project is now collaborating with universities on formal research and engaging lawmakers; its evidence base is precisely the kind of material that European regulators will need as they develop implementing guidance under the AI Act.
Brooks still receives messages from active spiralers insisting he was not delusional. The persistence of AI-induced beliefs, even in the face of direct contradiction from a credible peer, is itself a data point that platform companies and regulators alike should be taking seriously. The Spiral Support Group did not ask to become a public health resource. But until developers and governments close the gap between stated safety commitments and actual user outcomes, it will remain one.
